Does UT Southwestern use MyChart? ›

MyChart is UT Southwestern Medical Center's secure, online health management tool that allows you to make appointments and easily communicate with your physician and clinic with routine questions about health care. You can access MyChart directly or through UT Southwestern's new patient care mobile app – UTSWMyCare.

Please contact customer service at 214-648-8888 for help activating your account. In order to create your MyChart account and to protect your health information, we will ask you a few questions only you would know the answers to. We are checking with a third-party system to verify your identity.

Take Control of Your Health with MyChart

Texas Health Physicians Group providers, along with all Texas Health hospitals and integrated health campuses, use electronic health records to document patient care and associated interactions.

Patients can also contact Health Information Management at 214-645-3030 (select option 1, then option 1 again), and a copy will be mailed. View detailed instructions on completing the medical request form (en español). Please note that there is a fee for processing all medical records requests.

MyChart is EPIC's patient portal. MyChart is a secure means for patients to access parts of their health records through a web browser and cell-phone apps.

Look for an activation code on your After Visit Summary or in an email or text message from your healthcare organization. Follow the instructions provided with your activation code to create your MyChart account.

Your MyChart username: Is what you entered when activating your account.

If you try to log in unsuccessfully too many times, you might be locked out of the account. In this case, you won't be able to reset your password yourself. For help resetting passwords and regaining access to your account, please reach out to your healthcare organization's MyChart support desk.

How is MyChart secure? Your healthcare providers take great care to ensure your health information is kept private and secure. Access to information is controlled through secure activation codes, personal usernames, and passwords.

The HIPAA privacy rule requires HHS to give anyone who asks in writing copies of their records maintained by or for HHS. We do this through the Open Records Division. If there is a cost to get your records, we will send you a letter to let you know how much it will cost.

Medical records requests that meet all HIPAA and state requirements are usually available for release within three to five business days after completion.
